Medical Education Department of C L Gupta Eye Institute is an integrated department catering the educational needs of the institution and taking care of the training of long term and short term fellows, optometrists and nursing. It aims to provide its services in a manner that the institute’s vision and mission can be taken care of. With a faculty eager to teach and to transfer skills the trainees have an edge.

This institute has provided training to 32 ophthalmology fellows (national and international) including 6 long term fellows in the fields of cataract, cornea and refractive surgery, comprehensive ophthalmology, glaucoma, pediatric ophthalmology, medical and surgical retina. The Institute, also has trained over 500 students from India and abroad in various training programs for eye care personnel at different levels.

Training in (MSICS) Manual small incision cataract surgery

Phacoemulsification is not the only method for performing SICS. Manual small incision techniques for cataract surgery are also available and may be more suitable for developing countries. Manual techniques like the phaco section can be performed through smaller incisions, most manual methods require 5.5 mm or larger incisions. The major advantage of phacoemulsification over manual techniques, therefore, is the ability to implant a foldable lens through a 3.5 mm incision. Phacoemulsification is an expensive, machine-dependent technique that requires considerable investment and recurrent expenses. Compared to a manual small incision, the eventual advantage of a foldable IOL through a 3.5 mm incision is a 0.3D reduction in induced astigmatism. This advantage has to be weighed against the costs involved in doing phacoemulsification with a foldable lens.

Especially in a developing country like India where 51 percent of all blindness is because of cataract, the benefits of a paradigm shift to SICS are justifiable as it offers early visual rehabilitation, less chances of surgical astigmatism and better uncorrected visual acuity as compared to ECCE – and at a lesser cost when compared to phacoemulsification.

Course Outline

Supervised Surgery

The trainee will be guided through the steps of manual SICS with one-on-one supervision by a faculty member (The steps may not necessarily be done on the same case). The faculty member will assist the trainee and teach not only regular surgery but also management of complications as and when they arise. The complications encountered will likely include those specific to manual SICS. There will be discussions with the trainee before, during, and after the procedure.This shall be done for approximately 15 – 20 cases.

Independent Surgery

A faculty of the Institute will assist the trainee. The faculty member will be available in case any help is required. This shall be done for approximately 15 – 20 cases.

Qualifications

The applicant must be proficient Basics of Cataract Surgery (ECCE+IOL).

Selection

Selection is done on a first-come-first served basis. Some seats are reserved for candidates sponsored by NGOs.
